CONSULTATION TEMPLATE
Please Copy and paste into an e-mail so you are able to include images - send to consultation@Leviticus.com with your first and last name as the subject.
We are fielding all requests and new tattoo ideas as quickly and efficiently as we can. Thank you in advance for your patience.
Due to the finite amount of time our artists have for appointments, we are not accepting certain styles of tattoos such as bands around body parts, polka trash style, and large scale geometric imagery. If you are unsure if your tattoo falls into these categories, feel free to fill out a consultation form and we will let you know if we are able to take your project on. We are also limiting the projects our artists take on to imagery and styles they enjoy tattooing. While your idea may be great, it may not be a good fit stylistically for our artists. If this is the case, we will reply and ask that you understand we want to give you the best version of your tattoo even if that means declining a project our artists aren’t into.
This form is the starting point from which we will begin your appointment process. Please complete all pertinent areas as accurately and concisely as possible in order to process your idea and schedule your appointment as easily as possible. Once we have gathered all pertinent details of your tattoo idea, we will determine an appropriate amount of time for your appointment. When the consultation process is completed and we have given you the go-ahead to book an appointment, we will ask you call us to setup your date and time with a $100-$250 non-refundable deposit. We are unable to book appointments without all of the required information.

Requested Artist: You may list options, or we can help you choose an artist based on your idea. Some artists are booked out further than others, so let us know if you have a preference but are flexible to get you in sooner than later.
Preferred Time Frame: This doesn't guarantee a time slot, just helps us match you up with an artist that is able to accommodate you in the time frame you are looking
Budget: This helps us determine if we could do a more simple version of what you would like in your price range. On average, our artists charge around $170-200 hourly.
Tattoo Idea: Please include a description of your idea, along with at least one, up to seven images which represent the idea itself. Describe what you like and dislike about each photo and why the reference image is pertinent to what you are looking to get tattooed. You may find images to show size, but an approximate measurement in inches ensures it fits in the area intended.
Tattoo Placement: Ideally, the best resource for placement is a clear, straight on photos of the area you would like the tattoo. Avoid photos in a mirror as it gets confusing. You may even draw a shape around the specific area you are thinking or describe exactly what you are thinking to pinpoint the area and prevent further confusion. Including the size in inches is helpful as well. Oftentimes you will need assistance taking a photo as it needs to be taken from a straight on view, not angled. A blurry image or distorted angle will only serve to prolong the consultation process.
Lettering (if applicable): If there is lettering in your tattoo design, please send either the exact lettering requested in the specific words you would like, or find the exact font you’d like on a font generator. We use Dafont.com at Leviticus and have found it to be easy to use. Be mindful of a font’s limitations in regard to tattooing. Any letters with small loops such as a cursive “e” or “o,” or any small details such as those seen in “Old English” fonts may have to be simplified in order to create a lasting and attractive tattoo.
Cover-ups: If your tattoo idea involves a partial or complete cover-up, please send a clear photo of the existing tattoo and the area around it that should be included in the new tattoo.
Scar tissue: If your tattoo idea involves any scar tissue, let us know how old the scar is and include a clear photo of the area to be covered.
